Autumn Lake Heath Care at New Britain, a premier provider of subacute and long-term nursing care in Hartford County, is proud to introduce its highly specialized Pulmonary Care Program to the community.
Led by a Certified Pulmonary Disease Specialist who conducts weekly rounds, Autumn Lake Healthcare at New Britain’s trailblazing Pulmonary Program is highly effective in reducing the hospital readmission rate of patients with CHF & COPD by an average of 80% within the first 30 days of hospital discharge. In other words, patients are stabilized, restored to their highest level of functioning and assured a quick and safe return home – avoiding the headache of having to go back and forth to the hospital.
The facility’s expert nursing and therapy staff, which includes a full-time respiratory therapist, is extensively trained and well-equipped to treat high acuity pulmonary patients and individuals with chronic breathing conditions, as well as lung and breathing related diseases that include emphysema, COPD, pneumonia and asthma. The program includes tracheostomy care, weaning, decannulation, BiPAP, CPAP and trilogy.
“Our dedicated professional team is committed to the recovery and overall well-being of every patient, so it is no wonder that we are an industry leader in maintaining a low rate of hospital readmissions. Our patients are educated throughout their stay on managing their disease, and are able to return home empowered with the tools needed to maximize their health.” indicates Julie Popillo, Admissions Coordinator at Autumn Lake Healthcare at New Britain.
